The answer:

1. A function relates the two parts of the ordered pair to one another.

In your case:

the input of 0 is related to the output of 1
(you listed this twice in the statement of your problem)

the input of 5 is related to the output of 5

2. To be a function, only one output value can be assigned to each input value.

The ordered pairs (0,1), (5,5) do belong to a function

Determining the functional relationship:

Two ordered pairs is not much to go on.

One possibility is a linear function which passes through these two points:

f(x) = (4/5)x + 1
